Thank you for choosing to enroll in The Arboretum at Flagstaff’s 2019 Eco Explorers Summer Camp program! Camps are available for ages 4-11, with age-specific groups. Section 5: Parental Consent
Aware of risks*
- I am aware that while participating in a program or activity arranged by The Arboretum at Flagstaff, some risks and dangers may be present, including but not limited to those generally associated with certain activities, accidents, illness, and forces of nature.
Hold harmless*
- I agree to indemnify The Arboretum at Flagstaff and hold it harmless from and against any and all claims, suits, damages, liabilities and expenses, including attorney’s fees and The Arboretum’s costs of defense in connection with the loss of life, personal or bodily injury, and/or damage to or loss of property that arises from the participation of Camper(s) in the 2019 Eco-Explorers Camps. The Arboretum has my consent to secure treatment at the closest hospital in the event of a medical emergency.
Off-site consent
- In the event that The Arboretum schedules off-site field trips, The Arboretum has consent to take my camper(s) on off-site field trips, outdoor excursions, and hikes, including travel with Arboretum staff and interns in Arboretum vehicles as well as in staff and intern vehicles.
If consent is not given for the above, your camper(s) will remain at The Arboretum during scheduled field trips and excursions with supervision and limited programming.
Consent to film
- The Arboretum has consent to photograph, film, or quote my camper(s) for marketing purposes.
Consent to interact with plants
- Camp activities may involve touching and tasting edible plants. The Arboretum has consent to allow my camper(s) to taste and touch edible plants.
Consent to eat or cook plants
- Camp activities may involve preparing, cooking, and eating food. The Arboretum has consent to allow my camper(s) to participate in preparing, cooking, and eating food at camp.
Release*
- I release The Arboretum at Flagstaff and The Arboretum at Flagstaff’s personnel of any liability related to the administration of the over-the-counter, prescription, and emergency medication listed in this registration form. The Arboretum at Flagstaff’s personnel have consent to administer basic first aid treatment, for which they have been trained. The Arboretum at Flagstaff’s personnel have consent to perform CPR in the event of a medical emergency. The Arboretum at Flagstaff has consent to secure treatment by a local doctor or area hospital in the event of a medical emergency.

Section 6: Final step before checkout
Refund Policy: Refunds will not be given for cancellations made less than 14 days prior to the start of your registered camp. Cancellations more than 14 days prior to the start of your registered camp will receive a 50% refund.

Cancellation Policy: The Arboretum reserves the right to cancel any camp session due to insufficient enrollment or inclement weather. If this occurs, all fees will be refunded.
